Multiple Myeloma Presenting as Acute Kidney Failure Secondary to Lambda Light Chain Deposition

Renal monoclonal immunoglobulin deposition disease (MIDD) is a rare defined by of light chains and/or heavy on basement membranes and vascular walls the kidney. We describe case 71-year-old woman with kidney failure secondary to lambda in association plasma cell dyscrasia. Her initial serum protein electrophoresis did not demonstrate protein, classic cast nephropathy was absent renal biopsy. However, chain deposits associated changes confirmed MIDD. She achieved very good partial response (VGRP) after 8 cycles CyBorD (cyclophosphamide, bortezomib, dexamethasone) her function improved. This highlights importance an early diagnostic biopsy prevent end-stage disease. A review existing literature discussion management presented.
